  1. Actually, a B1 or B2 visa is good only for a maximum of 24 months (12 months + $100 fee = 24 months)

The only type of visa that I'm aware of that is good for 10 years is the "V" visa. This is limited to the spouse and children of a Lawful Permanent Resident of the U.S. (someone in the US on a green card), the purpose is so that families are not separated during the long period of time to process immigration paperwork for this classification of people.

So, it is very specific and not available to everyone.

  2. It's a B-2 tourist visa.  The maximum length of stay is 6 months.  The application cost is $131.  You can apply at any US embassy or consulate.  You might need an appointment so it's best to contact the US embassy or consulate in advance, before you go there.

All that's required is that you be able to demonstrate strong ties to your own country so that the consular officer is convinced you will not stay in the United States.
