Edit: Kanu is wrong, but can't find a 3rd player. Inamoto was at Arsenal when they won the title but never played a league game, so it can't be him.

Hm tough one...... All of these are still current players

Kanu: Champion with Arsenal, relegated with West Brom but never scored at a world cup.

Michael Carrick: Champions with Man Utd, relegated with West Ham, but never scored at a world cup.

Joe Cole: Champions with Chelsea, relegated with West Ham and scored against Sweden in the 2006 world cup.

Damien Duff: Champions with Chelsea, relegated with Blackburn and scored against Saudi Arabia in the 2002 world cup

There are many more who have done 2 out of the 3 but cant think of another current player to do all 3
