6 year old nephew- Wets himself, whilst awake...?

My 6 year old nephew just wet himself while he was awake, washing his hands...

He's not a bed wetter and it hasn't happened before, except on one occasion a good while ago.

His parents are split up and his father lives in England, he's in Ireland, and he's currently seeing a psychologist- he never had a definate routine growing up and everything just got on top of him, upset him etc...

He was also recently diagnosed with either ADD or ADHD, I'm not too sure which it was...

My two sisters (24 & 28) and I are the only ones in the house with my nephew, and thereofre the only ones who know about him wetting himself. I'm rather concerned about this...

Any ideas as to what could have caused it, or what's wrong?...

Again, not a bed wetter and only happend once before.

This happens all the time, its nothing, he's just excited or scared of something.

This is no big deal.  It happens to young kids sometimes.  I think you are getting too worked up.
