A company emailed me to advertise on my website...how do I organize a proposal?

I'm excited a modeling agency wants to advertise on my website, I decided on a monthly charge and now I'm trying to figure out what next? How do I organize a proposal to send back to them. do I send an order form, a list with what their getting using .pdf? An intro email, an example of what they are getting links to where the ad space will be, what the site will look like? Business Business...advice is truly needed here! Can anyone help?
