A letter to a dental patient to leave practice and find new dentist??????

I have to come up with a rough copy to send patients notifying them that they have been sent to collections due to delinqunt payments and to find another dentist within 30 days. How to word??????????????

I would simply tell them that their account has been sent to collections due to a breach in the financial responsibility (I am sure they signed some kind of form when they began treatment...)  and also say that because it has become a collection account, you can no longer treat them at your facility but would be happy to forward their charts and history on to their new dental office.


  2. Keep it Simple.

    This letter is to inform you of your final notice of delinquency in paying the amount owed for services rendered by (your office).  If not paid within 30 days time from postmark your name will be sent to a collections agency and we will respectfully decline your continued business.  After this point, we assume no further responsibility in assisting with your dental health and well being.
