A pre-certification phone?

I'm filling out a Health form for school.. its about health insurance.. then do i have to write a number of my health insurance company here?

They want to know who to call for insurance authorization in case you get sick or have an accident. The phone number is pre-printed on your insurance card. Yes, write the phone number on your form. The other questions probably ask for the insurance company name, address, your policy number, etc... Good luck at school.
