A programmer wrote a computer program for me. I need that person to sign off their rights to it, what form?

What form do we need to use to make it legal. the person needs to sign it and agreeing that I will now own it. It is not copyrighted by them, I am going to be doing that part. But I need to have the rights to it legally.

Just write/type something that say "I, "name here", giver all rights reversed/ownership of "computer program" to "your name". include the time and date and have them sign that.
