About filling out Canadian Visa Application Form IMM 5257?

Question

I am applying for a Canadian visa where I need to fill out its application form IMM 5257. In the uppermost part there is a box titled "File - Référence", could anyone tell me what it means? How should I fill it?

Also, what is common-law partner? Is it marriage partner or parents?

I am single so do I leave it blank?

My boyfriend live in Canada and i live in Jamaica, we meant three years ago he is still living in Canada we have not sent a full 12 month living to together. is that consider a common-law partner??
