  1. A W-9 FORM is a form where you put in  your complete information like tax id , name of the facility , address , phone numbers , etch...... This is in relation to the provider tax id that you were given before... If you move to different places or if you change any of the information.. you will need to submit that form so that it will be inputed in the database so that it will appear the updated information in the system...
