Ad agency wants to use my photo in a campaign—what do I charge?

An ad agency has approached me and asked to buy the usage of a photo from me to be used in a regional campaign including newspapers, magazines, web banners, and posters—pretty much everything except broadcast. I am an amateur photographer and have not sold photos in the past. The photo is very unique however, and through a little online searching I have found that the agency has already comped up an ad using my image and displayed on their website as part of their portfolio! I know they still need the high-res image from me to actually use it in a print campaign, and I also have figured out that the client has already picked my photo to use in the campaign. How much do I charge and how do I license the photo so I keep some control over it in case I want to use it again in the future?
