Add surname in passport?

Question

The surname filed in my passport, issued in india, is blank. Actually the firstname field in the passport is like x*x YYY, whereas it should be x*x in first name and YYY in the surname field.

How can i get it corrected?

 I have been in london i have indian passport   there is no surname in my passport. however, there is no surname in my other documents how i can add my surname in my passport

only my passport(surname filed is blank) and voter`s card carries my Given name only and no surname but my PAN card has my given name as well as my surname and in all my bank accounts it is my given name alon with surname .My husband expired recently so how can i show that i am the his successor as well as add my surname in my passport.My passoprt has only the given name of my husband in the "Husband Name" without the surname.
