Addressing a woman as "Sir"?

When, if ever, is it appropriate to address a woman as sir?

For example, would it be appropriate to address a women with higher authority such as in the military, martial arts, etc.?

    In my whole life time since the 50's, a woman in such authority was addressed as ma'am; never sir.

    One time I was sleeping in class, and my teacher taps me and im like 'SIR YES SIR' by mistake though.. and I got kicked out for the day..XD
