  1. So pleased you asked this question as I would also like to know. Most of my ceramic floor tiles are now boss as the underfloor heating was not laid correctly and many have started lifting. Nuisance and an expense I could well do without.

I used my dremel (a rotary tool).  I bought the bits (they come in different sizes to fit your grout line width) and an attachment that sets your dremel at an agle necessary to easily remove the grout.  I was surprised at how fast and thorough this method was.  I had "de-grouted" an average-sized kitchen in about twenty minutes.  Once you get the hang of using the attachment and getting the proper angle, It will work very smoothly.

I was totally happy with the process.

    good luck

  3. The best thing to do is to remove all of the old stuff and replace it with

Laticrete SpectraLock epoxy grout.  It is fantastic, doesn't loosen, or stain.

They sell grout saws that take a long time to get the grout out,  or you might be able to rent a roto-zip tool that will be quicker.

#2  Ceramic tiles loosening up is usually caused by too much flex in the floor or by improperly mixed (or too old) thin-set.  If your tiles were installed over electric "screen" type heat, it is possible that the thinset was spread too thin, or it's any combination of what I've mentioned.

The tiles will have to be pulled, floor checked and improved if there's too much flex, and re-laid with proper thinset with a flex agent added to it.

The SpectraLock would also help here.
