  1. No you don't have to specialize.  Most state require you to do a one year internship to be licensed.  This would allow you to be a general practitioner.  As far as  salary that would all depend on how hard you wanted to work.  My advise would be to get some specialty training and become board certified.  If you want general medicine then go to a Family Practice residency program.  That would take three years  after graduating from medical school.

No, getting an MD simply means that you're received your diploma upon completing med school.  What you do after that is your decision.  You never have to do anything beyond this, frankly.  If you wish to pursue clinical medicine in the US, then you'll have to do a postgrad. Residency in whichever specialty you wish.
