Airlines pilots have first class medical certificate. Does it mean that they have not had any diseases before?

  1. Airline pilots have at least a Second Class Medical Certificate.  Airline captains have a First Class Medical Certificate.

    That's just not possible.

    They are fit to fly untill the next medical.

A medical certificate simply provides evidence that a pilot has no common medical condition that might suddenly incapacitate him or otherwise interfere with his ability to fly.  It does not mean that he is disease-free or that he has never had any diseases.  Some diseases, such as epilepsy, diabetes mellitus, etc., can have an influence on the ability to obtain a certificate even if the pilot no longer suffers from them, until and unless it can be proved that they won't impair his ability to fly.  Past diseases like chicken pox or the measles are not considered important.

  4. No, it means that they are currently fit to fly the airplane.

  5. Nope - it really means they are certified to assist passengers with some medical emergencies.  It allows them to do procedures otherwise not normally allowed (tracheotomy).
