  1. You don't fill out a W-9 at a job, you fill out a W-4.  It's not age dependent, everyone has to file one regardless of age.

Your employer will send you a W-2 at the end of the year.  You use that to prepare and file your tax return.

A W-9 is to advise a payer of your SSN.  You will file one if you are self-employed and are paid by a business to do contract work for them, or if you open a bank account so that the bank can notify the IRS of any interest income you earn on your deposits.

  2. If you are an independent contractor, which you probably are NOT, you'd fill out a W-9 form.  As an employee, you'd fill out a W-4 form - the W-2 is the form your employer would give you at the end of the year showing what you made and what was withheld.

Neither has anything to do with your age.
