Am receiving Social Security Disability

Is this due to my doctors report on my medical condition or because I am receiving a small amount from my disability insurance from my former companies insurance.

SSDI is a government benefit and has nothing to do with your employers insurance.  I'm sure your disability with the employer helped qualify you for the SSDI though.
