American in Costa Rica need a Visa for Brazil?

Hi I am an American living in Costa Rica. I am planning on going to Brazil soon. I understand that as an American you are required to apply for a Visa before entering Brazil. But I was wondering what is required for the Visa application. Do I need to provide a bank statement? Is there a Visa service in Costa Rica that can help facilitate this process? Does anyone have experince with this matter? Any help and direction is appreciated.

BTW: Brazil is a phenomenal place, I've only been to the North East in Fortaleza, Brazil to visit family, but it's just amazing. Have fun =).
