Go to the website of US embessy in your country and you will find information

You need to contact the university of your choice. The requirements will be dependent upon many factors: your course of study, the country you come from (i.e. is English your main language), etc. Every college and university will list the exact requirements you need. At that point, you can begin to contact the embassy.
