Amplitude Modulation Math Question?

Here's my question (since I can't find the d**n formula how to do this):

An RF carrier of 10 V peak at a frequency of 15MHz is amplitude modulated by a baseband sinusoidal signal of 5V peak and frequency 5kHz. How do you work out the:

The modulation index,

The peak value and frequency of the sidebands,

And finally the peak value of the modulated wave?
