I believe the smallest size underwear available is 2T.  You should check what is available at Babies R Us, they may have something smaller :)

My best advice would be to find a website that sells products for EC (infant potty training) they usually have undies in all sizes. One example would be www.theecstore.com  but if you search for EC supplies, infant potty training supplies, baby underwear, elimination communication, and so forth you will find some.

They won't be found in any common stores, but they are out there.
