Ruby Tuesday has a salad bar, but at some locations it's right by the entrance which is pretty gross...

Pizza Hut dine-in restaurants do. Also, although it's not 'technically' fast food, Ruby Tuesday has a pretty big salad bar.

Yes, in my area there are a couple.  One is the Pizza Hut that has a salad bar for Lunch.  It's near the "downtown" eating area.  Also, there is a Burger King near a University that has one.  I think it's unusual to see one at BK, but I might be wrong.  I don't visit BK at all anymore.
