  1. YES,  I SEE IT EVERYWHERE!

  2. It's pretty much an unnecessary phrase, as everyone knows whatever has been said has been said.  I guess it would get annoying if repeated often enough.  In the past, I just may have been guilty myself of saying it, because it really doesn't define in any way, what has been said.  It's just one of those what I call, "tail-gate" expressions, similar to the excessive use of the word, "that".  How many times have you seen "that" in a sentence, when it doesn't have to be there?

"Mary told John that she went to the store, and that she bought something that she thought that her mother would like."

Here's the same sentence without "that":

"Mary told John she went to the store and bought something she thought her mother would like. "

Take out the four "that's" and the sentence says the same thing, only less cluttered.

  3. yes. it's arriving to cliche level.
