Anyone familiar with the ROTC scholarship secondary school transcript request form?

That's a mouthful. But if so, how much of it do I fill out and how much of it do I give my school counselor for her to fill out?

Looking over this form, you fill out the very top section - at least filling in blanks 1, 2, 3 and 4 (you can fill in 5 if you know your school's Educational Testing Service code), then you sign, just below, where you are authorizing the release of your high school transcript.

Everything else is filled in by the registrar at your school - your counselor will pass it on to the proper person. They'll send a copy of your transcript with a raised seal, an indication that it is an "official" school copy.
