  2. Normally, yes.  Short wave radio is all AM

  3. They can be, and the majority of them are.  SW, also known as HF (high frequency, 3MHz to 30MHz) can be AM, SSB, or even narrow band FM.  MW, or LF (low frequency) is usually used by commercial AM broadcasting stations.  You're really talking about two different things.  SW and MW are radio frequencies and AM, SSB, FM are ways to modulate (alter the waveform) with voice, music and etc.
