No, all rocks are not necessarily pushed to the surface: some simply recycle within the crust, by metamorphism and ultimately melting. If they all were brought to the surface they would have been eroded away and replaced with younger rocks - we still find very old rocks both at the surface and below the surface.

It all depends on the fault type the rock is on.  A thrust fault yes. Reverse fault or subduction zone no.
