Americans are fat pigs.

Yes.  It is rare, and most often the result of abuse, or from eating disorders.

I would imagine that a starvation death in the US as a result of not having the means (money) to have enough food to eat would be EXTREMELY rare.

However, eating disorders are common in the US and many anorexic girls have starved to death.

There are also parents and guardiens of children who intentionally do not allow children to eat as a form of punishment or to get the children to behave differently or do something.

Not really an easy question to think about.
