The above link is about the only place I could find that has even remotely ride-able waves in Italy. This looks like it is well south of Napoli, but it also looks like a cool place. Like the article explains, you need wind across the water for long enough distances to create a swell/waves, and it just doesn't happen in the Med. It is probably worse than the Gulf of Mexico or the US Great Lakes, but where there is wind and water, sooner or later you can have waves.

I have been told that the eastern Med, Israel and Lebanon, has some spots that are ride-able. There are a fair number of surfers in Israel, and I knew guys stationed in Lebanon who surfed there.

Spain and France both have some great breaks, and surfing is fairly popular in both countries.
