  1. a claim on what?? What rates?? Please write your question more clearly.

Our insurance company does not send stuff for prescriptions, but it will send stuff when I see the doctor since I am on my husband's insurance.  It will not say why I went to the doctor, but what doctor was seen and billed and when.  Your rates do not go up they are set with your policy.  We had United Healthcare Insurance company.

  2. I doubt it. Your insurance is supposed to cover medications. That's partly why u got the insurance. With out, you would have paid too much. What insurance company are u with?

  3. you pay a set premium for your health insurance. when you sign up for your health plan, you choose a level of coverage. the coverage stays the same for the length of your policy. the insurance will pay up to benefit level, and you are responsible for the remaining balance. for example, if your medication was $100 and your insurance covers 80%, you would be responsible for $20 of it

  4. no and no.  just go to the drugstore to get it filled, pay your copay.  that's it.
