  1. 1959 was when the STD system started

  2. STD (Subscriber Trunk Dialling) codes first went into service in the Bristol area in December 1958.   The call by the Queen referred to by the previous reply was to the Lord Provost in Edinburgh on the 5th of that month.    At the time, that was the farthest point from Bristol which could be dialed using the new STD system.

STD expanded rapidly during the 1960s, for example London subscribers got outward STD in 1961.   It took a good few years before most areas had both incoming and outgoing STD though, and some of the more remote, rural parts of the country didn't get it until the 1970s.

Prior to STD, calls could only be made without operator assistance to nearby exchanges by the use of local routing codes, generally a distance of up to 15 miles or so.   These local dialling codes were in use up until the 1980s.

  3. If I read your question correctly, you're asking when did the UK first start using STD dialling codes, as opposed to going via an operator?

I believe the initial trials were conducted in the early 1950's (53 to 55-ish) and the system was rolled out around 1959.  There is a classic piece of film footage showing the Queen making the UK's first official STD telephone call.
