Prophylactic IM ephedrine probably refers to use at the time of spinal anesthesia.  Some people do that to prevent the drop in blood pressure from the sympathectomy.  The trend now is fluid "co-loading", but it always helps to have a syringe of ephedrine or phenylephrine ready to go.
