I'm looking for articles, case studies, news reports, etc about anyone who has fallen from a height at work (i.e. window washers, power linemen, construction workers, etc). Specifically I need cases where the person was wearing a safety harness (or other fall-safety equipement) but was not rescued promptly or was not rescued at all. A hypothetical example story is "a window washer fell from the scaffolding and his independent lifeline kept him from falling to the ground. However, rescue crews were unable to reach him quickly because of the height at which he was hanging."
The accident may or may not be fatal, the most important thing is that the victim fell and was "saved" by their safety equipment. The outcome of the accident and the other people involved is relatively immaterial.
