Ok I don't know how to answer these questions

1. Proof of Physical- Private or school?

2. Proof of Health Insurance?

3. Associated Body Sticker?

4.Transportation Fee?

5. Signed Alcohol/Drug/etc. Free Contract?

6. Signed Sportsmanship Code of Conduct?

  2. the doctor gives you the physical.

    he or she checks if your physically able to play.

    your health insurance card.

    not sure about that body sticker

    transportation fee for away games.

    and the last 2 should be forms included in the packet of papers you got from school.

    1)  Proof of Physical: You need to get a physical- and you can either go and get one from your doctor or the school usually has a day with a doctor on site- and he/she examines you to make sure you're ok to play.

    2)  Proof of Health Insurance:  Provide any information you have regarding health insurance- this is for the school to keep so that if you were to suffer a serious injury they would know that you have medical insurance to cover expenses.

    3)  Associated Body Sticker- Not sure what that is- might have to do with whether you are a member of the Associated Student Body???  (ASB)??

    4)  Transportation Fee:  Fee to cover the money for bus to take you to away games, tournaments, etc.

    5)  and 6)  Are form that should have been included where you agree to their alcohol and drug policy and to their sportsman ship code of conduct.  Basically you won't drink, do drugs, or pick fights.
