You mean a rebate form for a cell phone you purchased from AT&T?  Normally they'll have a downloadable form somewhere on the listing for the phone in question.  It can be a different version form for different model phones.  If you want to let us know the model# phone your looking to get the rebate on, I can probably get a link for you to the exact page.

Notice that where it says "Mail-in Rebate Card" is clickable. Clicking that will bring up a link to the mail-in rebate form. As with most online forms it's a .pdf though so you'll need Adobe Reader. It's downloadable for free at Adobe.com, if you don't already have it on your computer.

I just finally recieved a rebate for a phone I purchased in December.  They sent it in the form of a $50 Visa gift card.  Make sure you keep the box the phone came in as well, because you'll have to cut out and mail the proof of purchase along with the rebate form.
