Try Grainger or MSC ( just like Grainger) if not find a local machine shop and they will be able to machine one for you.  You might mention what material you want the bushings in also.

True, I tried to buy some bushings for a mountain bike. King Bearing told me "they don't make metric bushing." I said, "what do they do in Europe?"

Well, if you ask for a mountain bike bushing, of COURSE they will say they don't have them!

Take a bushing to the shop and ask them to match it up. Just tell them it is a pivot bushing.
