The Ravello Festival is the oldest musical in Italy. It's not in Naples, but is on the Amalfi Coast not far away. It runs from the end of June to the end of November.

There's the Cornetto Free Music Festival at Piazza del Popolo in Naples in June every year. Sting performed in Naples last year as a part of the festival.
