  1. If there is no adjustment or tension mechanism you should be able to tighten by loosening the bolts that hold the rear wheel and slide the wheel towards the back which tightens the chain, then tighten the bolts.  This is common as the chain starts to wear a bit.  If it has already been adjusted all the way back, you can remove a link or two from the chain.  While looking at the chain from the side, you will see one or more that is a different color or shade that has a clip.  You open that clip and can remove a link in the chain.

  2. - Removing a link or two to put some more tension on the chain.

    - Moving the rear wheel axle back if you have horizontal/track dropouts

    - Buy a chain tensioner.

  3. Hey Gabriel,

You might also want to check your chain line.  If your chain line is off this causes the chain to throw itself off the front or rear cogs.  If you are using spacers on the rear hub body try moving the rear cog so it is perfectly in-line with the front chain-ring.  Or if you can't adjust your rear, you can try BB spacers and adjust your front bottom bracket if you have room for adjustment.

You can also try using a Half link if you are between a full length of a chain link for adjustment.  Half links can be purchased at any LBS.  Good luck.
