  1. Contact either The Bureau of Vital Statistics in the state in which you were born ( Usually in the Capital City) or at the County Clerks Office in the county in which you were born.(Located in the city that is also the County Seat )  Usually the fee for a certified copy of your birth certificate will be the same at either source.  Request are usually honored if requested by the individual or another person in the immediate family. You will receive a certified copy of your birth certificate and not the original  certificate. If you were hatched and not born then check at the hatchery where you were hatched....... Hey !  I gotta have some fun doing this. I know that there are three thousand comedians in this country out of work and I am trying to crack a joke but, just the same......

  2. Yes you need to go to the town hall and apply for another one if you live in the UK, yes it will cost you to get a new one, in the mean time you should report it to the police in case someone recover it and mess use it.

  3. It depends on your state but typically under $20 for a copy. You get them from the vital statistics/records office in the state you were born.

  4. If you are in the UK you can either order one from the registry office in the town where you were born for £7 or from the General Register Office (but for the later you will need a reference number or it will cost more). Don't Google "birth certificates" go straight to the General Register Office website, or you will end up paying a lot more.

  5. UK answer.

You can order either a short or full copy of the birth certificate from the register office. It is £7:00 for the full copy for certain, but I have never ordered the short copy, but I believe it to be about £5:50.

You do not need the GRO numbers that are included in the BMD index for a register office order.

You can as the other person has said get a copy of the certificate from the  GRO (General Register Office), and for this you will need the GRO numbers (volume and page) They cost £7:00 for an  on-line order, and £8:50 for a phone order. The service you get with the GRO is excellent.  You should receive the certificate between 5-7 working days after the order.

    Hope this helps.
