  1. Well, it's pretty hard to give a complete answer because there is a lot of information that isn't clear here, but I'll try to give you some general information. The brain is encased in a non-flexible compartment - the skull. When the brain is injuried, just like any other part of your body, it swells. Butwhile the brain swells and gets bigger, the case it is in doesn't enlarge, so there is added pressure on the already injured brain. The swelling is from fluid that accumulates as part of the healing process. It would be just like getting a cut on your arm. It swells up and is painful for awhile as it is healing. The treatment would depend on the cause of injury, but the initial response in a critically ill patient would probably be to give him or her Mannitol, which is a very strong sugar solution. The sugar pulls the extra fluid out of the brain and helps decrease the swelling. Depending on the location and amount of swelling, sometimes a neurosurgeon will cut a small window out of the skull to give the brain some more room to get bigger with the swelling. If you can write again and give some more information about what caused the swelling, I can be more specific. I hope she is doing well.

  2. usually if its really bad, they have to drill holes in the skull.

    but only to release pressure?

i am not sure, don't take my word for it.
