  1. according to the NEC the panel must be no higher than 6ft from finished floor or grade to the top of the highest switch in the panel.  industry standard is 6ft to the top of the can.

  2. If I'm not mistaken, the standard code would vary depending on location (geographical).

TAKE NOTE:  To all readers, be advised that it's highly recommended that you use a licenced and experienced electrician, for the installation and wiring of a new breaker box.

  3. Assuming that your Main Disconnect is at the panel, The National Electrical Code requires that the Main Disconnect be no more than 6'-7" above the finished floor [2005 NEC Article 404(A)] (this was changed in the 1999 NEC, it used to be 6'-6";  6'-7" equals 2 meters, so the Code was changed in an attempt to be more "International"). The same would apply to an exterior Main Disconnect, except that you should check with your local Inspector, Fire Department & Utility Provider for any Amendments to this Article.

The Disconnecting Means shall not be located in a bathroom or in a closet, must be readily accessible, and must be suitable for the use [Article 250.70 (A) & (C)]. Adequate working space and clearances must be provided per Article 110.26, also.

I'm glad to hear you are hiring a qualified, licensed contractor to do the work; be careful, just the same.
