CMS-1500 Form?

Question

Does anyone know where I can find a OMB-0938-0999 Form CMS-1500 (08-05) that I can fill in on the computer instead of having to fill it out by hand?

Or is that a HIPPA violation?

However, it cannot be saved on your computer--you will need to print it and scan it...
