  4. The presence of HIV DNA in breast milk has been documented in many studies. Yes, transmission can occur through breastfeeding.

    Evidence to support this include:

    -isolation of HIV from cellular and non-cellular fractions of breast milk

    -case reports of infected children who were breast fed by mothers who acquired HIV infection postpartum (after giving birth)

    -documentation of transmission of other retroviruses (HIV is a retrovirus) through breast milk

    -and reports of breast-fed children who were initially HIV negative but became HIV positive during breast feeding.

    The exact mechanism of transmission through breast milk is still not well understood, and clinical, iimunologic, and virologic factors in both mother and/or infant likely play a role. It has been hypothesized that transmission of HIV through breast milk may be facilitated by the higher susceptibility of the immature neonatal gastrointestinal tract, which may be particulary permeable to HIV. The risk of infection could increase further with mucosal damage, which may be associated with the early introduction  of other liquids, types of milk, and with gut infections. It is possible that tonsils may also serve as a port of entry for HIV-1 in breast milk.
