  1. No, these days each person travelling must have their own passport.  So, that's one passport for you and a seperate one for your wife.

  3. Only if she's also your child and under the age of 16 years.

    Which I suppose is a bit unlikely, even in The Netherlands.

  4. As mentioned I'm afraid that is only a (previous) option for kids up to 16 years old.  Now even they are required to obtain a passport (at least according to my local gemeentehuis (town hall).

    If your wife holds an American passport then she can usually obtain a passport relatively easily and quickly from what I have read.This is sufficient for her to travel to the Netherlands for tourist travel up to 3 months under the visa waiver program.  She will not need a visa in this case.

    Therefore this is your easiest option.
