  1. it depends on the airline some allow it, some don't, some only allow it on certain flights.  You really have to go sit down with a travel agent OR call every airline and individually ask them.  I'd recommend the travel agent.  They'd know exactly who to call and how to find the information quickly as opposed to you calling, being on hold then being transferred to the wrong department 20 times.

That is a question you will have to ask your specific airline.  Some airlines will allow pet carriers- such as American.  Southwest will not.  And American will only allow it if the carrier can fit completley under the seat in front of you.  You cannot put it in a seat, hold it in your lap, or put it in an overhead bin. The charge is somewhere around $100.
