Can I get from La Guardia to JFK between midnight & 5am on New Year's morning to make a connection? How?

I have to take a flight that arrives in La Guardia at 11:58pm on New Year's Eve and departs from JFK at 5:20am on New Year's Day. How can I connect between the two airports in the middle of the busiest night of the year?

Follow up: On the way back, I arrive at JFK at 1:30am and depart from Newark at 7:35am (this time on a normal Wednesday). Is this even possible??

You should go to the airports' websites and check for "ground transportation".  You may find that taking a cab is the best way, but that will be expensive [and a long trip] between JFK and Newark, but there shouldn't be much traffic in the middle of the night.  I wouldn't think it will be all that "busy" between LAG and JFK over New Year's Eve into NYDay - everyone will be busy partying, not driving between airports.
