Can I obtain passport for honeymoon trip with a "short form" wallet sized birth certificate?

I have a wallet birth certificate that's been in my wallet for quite a long time. It is an original but has a crack going across it on the lower half of it. It does not cut out any vital info however. Birthstate is Virginia so I don't think I need the "long" form but would they except my cracked wallet one?

No you can't.  I had a wallet size birth certificate also and they refused it.  There is a size limit on the document that they will accept.  It's been a few years, but I think it has to be 6x8 inches or larger.
