In Ohio, a work permit is required for people aged 14-17.  The application must include a medical record, a signed agreement from the employer, and the parents' permission.  Here are the rules:

What kind of workers permit? The minimum age for a job is actually 14, but you can only work a limited number of hours each week. If you are not at least a permanent resident of the US I don't think you can get one.
