Can a community nursery nurse sign my sure start maternity grant form?

I didn't know this grant was available to me and i was discharged by my midwife. the community nursery nurse is due to pay me a visit on 21st august, will it be ok if she signed it? or does it have to be a midwife or health visitor?

It has to be the person who delivered the baby or someone who saw you throughout the pregnancy

my health visitor told me yest that she can help with applying for this and other benefits so give her a call and I'm sure she'll be able to help u out :)
