Can a parent notarize form DS 3053 before...?

I need my husband to sign and notarize a statement of consent form (DS 3053) agreeing on the issuance of a US passport when the baby is born. Can he do tha before the baby is actually born or does he have to wait till the baby is born for the form to be notarized? cuz he is not in the states with me and wont be there when the baby is born.

  1. The form requires the child's date of birth. How would you know this ahead of time?

    i have a friend i could your e mail to her she lives in america and i m sure this is very close to her field of working

    what you need is to send me your private e mail and i would send it to her

  4. Wait till the baby is born, then have him fill in and notarize the form at a US embassy or solicitors office abroad.

    Then mail the form to you.

    This will be fine.

  5. am sure you cant, you should have a certificate from the hospital with , name, date of birth, place of birth

    and all that as you know necessary to issue a passport.
