Don't answer if you don't know. Don't cut and paste information. Thanks in advance.

  1. noooo

  2. The pharmacist would need a legitimate reason to look up that information:

    1. The patient requests the data.

    2. Another privileged entity (the patient's doctor) may request information from the pharmacy.

    3. Some other legally recognized proxy (e.g. power of attorney) requests the data.

The pharmacist or pharmacy technicians cannot just randomly look up information on a patient. Patient privacy and security is protected by the Health Insurance Portability and Accountability Act (HIPAA).

Not even other family members can request that information, unless prior permission is given by the patient. Doctor's and other privileged entities can request that data without the patient's expressed knowledge. However, those kinds of transaction have to be listed in the HIPAA document they sign when they visit their doctor.

That said, when a valid request is made, the pharmacy generally requests a name and then verifies an address or date of birth. They *should* be checking for some form of identification, but this is up to the individual pharmacy's workflow.

  3. no ... the pharmacist would need more information than just a name ...

  4. Am I allowed to cut and paste my answer since you asked the same question twice?

IF you are in the pharmacy's files and they look up your name, the pharmacist CAN access your drug information and any insurance information you have on file.

If you are asking can a pharmacist access your insurance information and bill your prescription to your insurance only knowing your name, then the answer is no. They would need to know the name of your insurance company, your ID number and group number (provided to you by the insurance company) your relationship to the subscriber, if the insurance is through someone else like your parent or spouse. You must provide this information to the pharmacist, they have no way of accessing the information with just your name. (Just as if you walked into Sears and told them to bill your new washer and dryer to VISA but didn't have your charge card, or number, etc... There would be no way for them to "access" your VISA info to bill your account just by giving them your name.)

[edit] Please clarify your question. Are you asking if a pharmacist has a reason to ask you for more information than just your name to look up your information? They usually ask for a date of birth or phone number to make sure that they are accessing the correct files in case someone has the same or similar name to yours. Will they answer your questions or provide you with any of the information on file if you don't provide any other personal information or ID? No, because that would violate Federal Privacy Laws and they cannot give out that info or discuss it with you unless they are sure you are the individual named.
